The documents that are considered in this regard include: Court order granting sole legal custody to the parent applying for the child passport Consular Report of Birth Abroad, Foreign or US birth certificate and adoption decree, where the applying parent is only listed as the sole legal parent/guardian of the child Judicial declaration regarding incompetence of the non-applying parent/guardian Court order permitting specifically the applying parent’s/guardian’s travel with the kid/minor Other parent’s/guardian’s death certificate When both guardians/parents are not available Though this case is rare, but it might happen and hence there is a clause with the US State Department for the same. A third party is allowed to apply for the child passport, but with some documents in place. Both parents/guardians have to provide an affidavit or a statement which will be duly signed and notarized. The statement will provide authorization to a third party for applying on behalf of the child. The parents’ or guardians’ photocopied identification documents have to be attached to the affidavit or the statement. If the statement comes from one parent only, sole custody of the parent has to be established through valid documents. Step 3 - Visiting the application acceptance facility This is the last step as how to get a child passport in USA. After the form is filled up and all necessary documents collected, visit the nearest passport application acceptance facility.
